Dear Reader,

The file addendum.doc contains three tables that were inadvertently not 
included in the first printing of Hosmer, D.W. and Lemeshow, S. (2000). 
Applied Logistic Regression: Second Edition, John Wiley & Sons Inc., 
New York.

The file has been formatted in such a way that when it is printed the 
pages may be easily inserted into the text.  Second and subsequent 
printings will include the tables as an addendum.
